The Company reserves the right to conduct Pre-employment, Return-to-Duty, Pre-Transfer, Random, Reasonable Cause/Suspicion, Post-Accident, Scheduled-Periodic, Post Rehabilitation Follow-up, and Pre- Access testing for drugs and alcohol. Refusal to take a requested drug test is automatic termination.

All employees are responsible for their own transportation to and from the rig.

I agree to furnish any and all such additional information and complete any and all examinations or testing, as may be required to complete this hiring process and provide complete documentation for my personnel file.

I understand and agree that if I am hired, there may be a Benefit Waiting Period, during which time certain benefits will not be available.
