Who Is This Program For?

The Mindful Mentoring program is an exciting all-inclusive program being offered through Big Brothers Big Sisters of Central Vancouver Island (BBBSCVI).

The goal of the Mindful Mentoring program is to provide participants with the opportunity to improve their physical, mental and emotional health, and build healthy relationships through learning about yoga, mindfulness and psychology.

The Mindful Mentoring program is completely free and open to participants ages fifteen to twenty-two.

Program Details

• The program runs once a week for 7 to 10 weeks.
  
  
• The first eight classes will focus on learning about yoga, mindfulness and meditation. These classes will involve some lessons on psychology and personal reflection to better understand the mind and how exercise, mindfulness and meditation can be used for personal development.
  
  
• During the final two or three classes, two Cognitive Behavioiural Therapists will present participants with tools they can use to improve and maintain their mental and emotional health.
  
  
• Snacks and drinks will be offered at every class.
  
  
• As part of the program, participants will be asked to complete an evaluation form telling us about their experience with the program. This information can be used to make program improvements and determine the impact of the program has had. You can complete the survey anonymously, and the information collected will be strictly confidential.
  
  
• Most weeks we will email a summary of what we talked about, along with a small reflection or practice to do during the week.
  
  
• The exact dates and time of the program will be texted and or emailed to participants.

Informed Consent (to be completed by youth/participant)

I hereby give permission to Big Brothers Big Sisters of Central Vancouver Island (BBBSCVI (the “Agency”)) to provide their service to me. It is my understanding that the intention of the agency is to offer me an opportunity to participate in a group program led by a responsible individual for the purposes of shared activities, friendship and support. I understand that all efforts will be made to select a responsible mentor who will facilitate the group program.

In consideration for this service and other valuable consideration provided to myself by BBBSCVI, I release the agency of all responsibilities and liabilities in connection to their services provided in good faith, to myself. I permit the agency to release any relevant information, including my personal information, to Big Brothers Big Sisters of Canada and their insurers, as may be appropriate in connection with any legal proceeding, inquiry or risk thereof.

I understand that the collection of personal information about me will be held in strict confidence and is to be used solely for the purposes of administering the program. I further agree that information about me may be shared, at the discretion of Big Brothers Big Sisters, with the group facilitator so that my needs may best be met. 

I understand that this application is the property of BBBSCVI. I also agree to participate in the Pre-Match Training Program administered by BBBSCVI to be provided prior to participation.

I HAVE READ AND UNDERSTAND THIS AGREEMENT. BY SIGNING THIS AGREEMENT, I ACKNOWLEDGE THAT:

I, named above, hereby request Big Brothers Big Sisters services for myself. I am aware of and understand the risks, dangers and hazards associated with the above service and agree that these service are suitable to me.

Media Consent

We want to know if it's okay for us to take photos and/or videos in which you are featured or can be seen, to be used in promotion of our programs. Please read through the agreement as seen below. You don't need to consent to participate in this program!

I hereby grant permission to Big Brothers Big Sisters of Canada (National Office) and its associated member, Big Brothers Big Sisters of Central Vancouver Island (Local Agency), to use any photographs, audio, and/or video recordings of my child or youth. These media materials may be taken or produced by staff or contracted media personnel, and/or National Office or Local Agency staff during regular scheduled group programs, special events, or outings associated with the group programs in which my child is enrolled.

I consent to the use of these media materials by the Local Agency and/or the National Office for promotional purposes. This includes but is not limited to brochures, posters, newsletters, media information, advertisements, audio-visual productions, and digital media platforms (such as local agency websites and social media channels).

Furthermore, I understand and agree that these photographs or video productions may also be shared with community and school partners for the purpose of program promotion.

I acknowledge that this consent is given freely and without expectation of compensation or remuneration now or in the future.

Parental Consent (to be completed by the parent/guardian)
