  • Scholarship Awards & Annual Volunteering

This section must be completed by all families enrolling their child at Fellowship Christian School. Volunteer hours will be invoiced as an annual fee ($270 for full-rate families and $540 for reduced rate/scholarship students). If families choose to volunteer, the annual volunteer fee will be waived in lieu of successful completion of volunteering. Families paying full rate must volunteer a total of 1 hour per month (10 hours total, per school year). Families receiving an available scholarship are required to volunteer 30 minutes per week or 2 hours per month (20 hours per school year). The annual volunteer fee is due October 1, 2024. Some volunteer hours are offered/start during the summer and are open based on the volunteer calendar/needs of FCS. If a family is volunteering in lieu of the volunteer fee, they must sign up for the needed slots as this helps ensure FCS does not need to hire for a specific duty they are in need of. If a family does not see a task/slot they are willing to fulfill, the annual volunteer fee will need to be paid, but you are still welcome to volunteer (with 48 hours notice) in other approved areas not listed on the volunteer form (as a reminder, this will not cancel the annual volunteer fee, though).

  • I understand that I am required to be reliable and show up for all required volunteer days.  I also understand that should I not be able to attend a scheduled volunteer day, I will be required to give at least one week notice and will not receive credit for the missed day. Missing and/or cancelling more than one day will result in automatic required payment of the annual volunteer fee being due as FCS will not be able to use my volunteer time in a scheduled manner.

I will sign up for all required hours by no later than August 15, 2024. I will fulfill my signed up slots/hours each month to fulfill the annual volunteer requirement. I understand volunteer hours can not be filled within one month and must be distributed throughout the entire school year (each month), allowing FCS to have a reliable person scheduled for each need throughout the school year.

  • Annual Fundraising Obligation

The obligation is per family and may be raised through school fundraisers and/or independently and donated to Fellowship Christian School. Details are provided below.
  • I agree to complete the Fellowship Christian School annual fundraising obligation. I also understand early withdrawal/unenrollment will still require completion of the annual fundraising obligation.

  • I agree to complete the Fellowship Christian School annual fundraising obligation. I also understand early withdrawal/unenrollment will still require completion of the annual fundraising obligation.

  • Annual Obligation: Each family is obligated to raise $350 per family. The fundraising obligation must be completed no later than March 15, 2024. The annual fundraising obligation will allow FCS to raise necessary funds for the ministry without raising annual tuition rates as well as help spread the word in the community. Families will need to raise the money on their own (independently) and are also able to raise funds through any annual school fundraisers (Dinner, Auction & Talent Show, etc.). If the annual obligation is not met, families will be required to pay the balance of the fundraiser amount not met by March 15, 2024.

    Past annual fundraisers have included:

    • FCS Candle sales
    • FCS Krispy Creme Donut Fundraiser
    • Annual Dinner, Auction & Talent Show
    • *New this year* - Annual Raffle

Annual Raffle Obligation: FCS will also be hosting an annual raffle. The first 25 raffle tickets sold by each family will not count towards the required annual family fundraising obligation, but any additonal raffle tickets sold over 25 will count towards the annual family fundraising obligation.

All FCS families will be required to sell 25 raffle tickets each (at $20 each). Two prizes will be given out. One prize will be free tuition for one student for one school year. This can be applied to the current school year or the next school year. The second prize will be an item to be determined by FCS. The raffle drawing will take place at the annual Dinner, Auction and Talent Show (typically held in March).

Please provide any pertinent medical information, including any/all student allergies.

  • I understand that shoud I give permission for my child to take any of the above medications/medicines, I am required to purchase and turn in the labeled medication to the front desk/office with my child's name on it (medications are not allowed to be kept on/with students or in their backpacks). I understand it must be in the original container as well as be enclosed in a labeled, clear, ziploc baggie. • Commitment

    Faith Based Teaching Agreement
  • Fellowship Christian School admits students of any race, color, national and ethnic origin to all the rights, privileges, programs, and activities generally accorded or made available to students of the school. Fellowship Christian School does not discriminate on the basis of race, color, or national and ethnic origin in the administration of its educational policies and admission.

Fellowship Christian School’s biblical role is to work in conjunction with the home to mold students to be Christ-like.

I wish to register my child for attendance at Fellowship Christian School for the current school year. I have read the Statement of Faith and agree that my child may be taught according to the doctrinal basis of the Bible while attending Fellowship Christian School.

    Tuition, Fees & Enrollment Agreement
  • Enclosed or provided is the annual, non-refundable registration fees, which I understand are refunded only if a student is denied admission by the school.  I understand that without the paid fees, my child will not be placed on the list for enrollment nor held an enrollment spot.  Fees are due immediately.

I understand that in signing this enrollment application for the coming academic year, I am agreeing to accept the rules concerning payment of tuition and fees as set by the Tuition & Fees Policy of the School. I understand that all fees and rates are annual and I agree to pay in full or split the tuition into 10 consecutive equal payments/installments, starting no later than August 1, 2024 and commencing no later than May 1, 2025.  I understand all fees are non-refundable and I also understand tuition is not pro-rated based on the number of days my child attends school or the number of school days in a month or for any other reason.

I understand that in the event of withdrawal/un-enrollment, the balance of outstanding fees and tuition must be paid before clearance is given to other schools in the transfer of transcripts/records. I understand that If I choose to withdraw my child, I am required to and will give at least one months (30 days) written notice (handed into the school office) and pay for the fees associated with the withdrawal/un-enrollment of my child (no less than 2/10 of annual tuition or 2 monthly tuition installments).  I understand I will continue to be billed and incur fees until I turn in this written notice to the school office.  I also understand un-enrollment of my child during the month of April, May or June will result in no pro-ration to the annual tuition and fees due.  April, May and/or June un-enrollment will not reduce the annual rate or fees due.  Full fees and tuition must still be paid.

Lastly, by signing this agreement, I understand that I must pay at least the first three installments of tuition (annual or monthly) regardless of whether I choose to unenroll/withdraw my child early. The reason for this requirement is to make sure families that are waitlisted do not miss an enrollment opportunity due to families not being commited to enrollment decisions. We understand there are life changes and this is why we allow unenrollment, but we do also say no to other families when a spot is taken, often times leaving other families to make other plans for their child's education, resulting in the loss of that enrollment/opportunity for that family.
