We are surveying Service Coordinators at Regional Centers to discover potential needs for additional and ongoing coordination of care. Our survey focuses on the need for a whole person assessment. A whole person assessment is a comprehensive evaluation of all aspects of an individual's life. This includes medical, mental health, social, economic, religion, and many other factors. The purpose being to create a plan of support that meets most if not all (within the scope of the available resources) of the individual's person centered wants and needs.
