This form should be completed once (1) for each child participating in the 2024 Summer Reading Program, "Adventure Begins at Your Library." If your child is younger than the grades listed on this form, please refer to our summer calendars or visit the "Preschool Programs" section of our "Kids' Summer Reading Program 2024" webpage to view summer offerings for our readers-in-training. For reporting purposes, this form cannot be completed for children younger than 1st grade.
