Waiver & Consent
1. I confirm that I own the pet or I was given authority by the owner for taking ownership of the pet.
2. I assure Sniff Scratch Snooze Kennels that this pet is currently in good health condition.
3. I confirm that my pet has a complete and updated vaccination.
4. I acknowledge that the pet should have a safety collar.
5. I confirm that my pet doesn't have any flea or any communicable disease.
6. I release Sniff Scratch Snooze Kennels, its employees, and owners from any liabilities but not limited to injury, sickness, damage, accident, or death while in the vicinity.
7. If ever that I wasn't able to pick up my pet on time, Sniff Scratch Snooze Kennels will be calling the owner and get authorization for the pet to stay for a pre -agreed extended period.
8. If my pet needs medical attention, I authorize Sniff Scratch Snooze Kennels to have it examined by a veterinarian.
9. If you will avail of our service during the holidays, please make sure to reserve well in advance .
10. I confirmed that all information I entered in this form is true and accurate.
11. I give consent for SNIFF, SCRATCH, SNOOZE kennels to take and post photos/videos of your pet for business purposes only.
