Dear Owner(s):  Basic Obedience class teaches basic on-leash commands and manners (Sit, Stay, Down, Come, Off and Leash Manners).  Practicing and using these commands as part of your dog’s everyday life is your responsibility and is essential for your dog’s training.   Basic vaccinations are required to be kept up to date.  By signing below, you agree to the following WAIVER AND RELEASE OF LEADER OF THE PACK:

Animal behavior can be unpredictable and participation in Leader of the Pack training, education or obedience classes is not a guarantee of your pet’s behavior at any time.  Payment of class fee is due at the 1st class and is non-refundable.  Training services are provided as available and appropriate, as determined by Michelle Bowersox. 

 I agree:

To release and hold Michelle Bowersox and/or Leader of the Pack LLC harmless for any and all claims of any kind, including bodily injury or damage to ourselves, our dog, other persons, animals, or property, whether such injury or damage occurs during or after any Leader of the Pack training.

To further release and acquit Michelle Bowersox and/or Leader of the Pack LLC, and/or all other persons for any liability for medical or veterinary expenses that may be a result of any accident or incident that may occur.
 

To not withhold or alter any information about this dog's behavior from Michelle Bowersox, and agree to report all incidences of aggression to Michelle Bowersox during the duration of training.  Client authorizes Trainer to share information, including photos, with Client’s veterinarian and/or other trainers.

 
To allow video and photos to be taken and shared online as deemed appropriate by Leader of the Pack, LLC.
 
By entering into this agreement, Owner(s) expressly acknowledge that no verbal representations made subsequent to the signing of this agreement shall vary or in any other manner alter the terms and conditions of this agreement.  ____________
