Tell me about your desired custom tattoo! Please be as descriptive as you can. What components are you hoping for? PLEASE INCLUDE APPROX SIZE and any other info you feel I might need to design this tattoo!

EX: I would like to get a vintage NIKE shoe with a purple swoosh! I would also like some 90’s shapes around it! Partial color, around 4 inches (OR) I’m looking to get a T-REX dinosaur wearing roller skates, black only, about half my inner forearm!

Please attach one well lit photo of the area of your body you’d like the tattoo to end up. Please include any nearby or conflicting tattoos, moles, scars, or any other skin concerns. It is better me if someone else takes the photo (EX: you’re getting a bicep tattoo, take the photo outside in good light in a tank top, standing naturally, and have another person take the photo directly a few feet away.) Photos taken in a mirror are generally too contorted to be helpful.

Please include any references (less than 7) for your tattoo. These will help me to understand what you want your desired tattoo to look like! These can be photos, drawings, paintings, tattoos by other artists, my tattoos/flash etc… Great places to find tattoo references are Instagram, Pinterest, Flickr, or google!

Any allergies/skin conditions/health concerns I should be aware of?
