A USTA League Team Confirmation & Agreement form is required for all USTA League teams. Captains need to complete the information listed below and sign the form. Captains must digitally sign this form for the team to be included in the match schedule. Captains need to acquire confirmation (verbal, email, or written) from your facility prior to submitting this Agreement.

Completed Team Confirmations must be submitted before January 27, 2026 to be included in the league season.

A team number will be sent out after this form is submitted. Use the team number provided by the Regional League Coordinator to add players to your roster.

Important Dates:

January 27 - Team Confirmation Due

February 3 - At least 6 players registered on your team

February 10- Match schedule available on TennisLink

February 20 - Local Season Starts

May 10 - Local Season Ends

May 16-17 - Playoffs if needed
