Good news! We have already created an account for you on our Patient Portal to complete your registration.

Logging on to the portal is simple and can be done using a secure login code sent to your phone after you enter your identifying information (Name/DOB) on the Patient Portal Login Page .

Once you're on the Login Page (after clicking the yellow button to get to the page) make sure the 'Using Mobile phone' setting is selected to get the code.
