Pet Boarding Agreement Form

This Pet Boarding Agreement (the "Agreement") is entered into between Lex's Coastal K9 training also known as Coastal Florida k9 services ("Boarding Provider") and the pet owner ("Client") whose details are provided above.

4. HEALTH AND VACCINATIONS

The Client confirms that the pet is in good health and has received all required vaccinations. The Boarding Provider reserves the right to refuse boarding if the pet appears to be in poor health or if vaccination records are not provided.

5. EMERGENCY CONTACT

In case of emergency, the Boarding Provider will make reasonable efforts to contact the Client. If unable to reach the Client, the Boarding Provider is authorized to seek emergency veterinary care for the pet. All associated costs will be the responsibility of the Client.

6. BEHAVIOR AND TRAINING

The Client acknowledges that the pet will be housed in a shared environment with other animals. The Boarding Provider will make reasonable efforts to maintain a safe and secure environment, but the Client accepts responsibility for any behavioral issues or damage caused by the pet.

7. MEDICATION

If the pet requires medication during the boarding period, the Client must provide detailed instructions and sufficient medication for the entire stay.

8. CANCELLATION POLICY

Cancellations must be made at least [number of days] days in advance. Failure to provide sufficient notice may result in a cancellation fee.

9. AGREEMENT TERMINATION

The Boarding Provider reserves the right to terminate this agreement at any time if the pet's behavior poses a threat to the safety or well-being of other animals or individuals.
