Liability Waiver and Release Form

I,   First Name   Last Name   , acknowledge that I have voluntarily chosen to enroll my dog,   Type a label , in training sessions with Lex's Coastal K9 Training.

In consideration for the training services provided by Lex's Coastal K9 Training, I, on behalf of myself and any other parties claiming through me, hereby release and discharge Lex's Coastal K9 Training, its owners, employees, trainers, and agents (hereinafter referred to as "Releasees") from any and all claims, liabilities, demands, actions, causes of action, costs, and expenses, whether at law or in equity, whether known or unknown, arising out of or in connection with the training services provided.

I understand and acknowledge the inherent risks associated with dog training, and I assume full responsibility for any injuries or damages that may occur as a result of my dog's participation in the training sessions.

I further understand that Lex's Coastal K9 Training is not responsible for any injury or damage caused by my dog to other dogs, individuals, or property during the training sessions.

I agree to indemnify and hold harmless Lex's Coastal K9 Training and its Releasees from any and all claims, demands, or actions arising out of or in connection with my dog's participation in the training sessions.

I have read and understand the terms of this Liability Waiver and Release Form, and I voluntarily sign it as my own free act and deed.
