  • I hereby affirm that all the information provided above is true to the best of my knowledge. I authorize The Tax Lady LLC. to prepare my taxes based on the information provided. I understand that I am responsible for any outstanding balance due to any false information given. If there is a balance due, the tax preparation fee must be paid upfront. In case an entity takes the entire refund, I am still responsible for paying the tax preparation fee for services rendered. Failure to pay the tax preparation fees will result in legal action taken by The Tax Lady LLC. to collect the unpaid fees.

     

    If an amendment has to be filed with no fault of the tax preparer, an upfront fee of $950.00 is required. The Tax Lady LLC. will email a copy of the return once it is in "accepted" status with the IRS and the state. Any copy after the first copy will incur a fee of $50.00. All fees, including bank and service fees, are non-refundable and non-negotiable. Please use www.wheresmyrefund.com to track your federal refund and the state Department of Revenue site for any State Refund tracking. Please do not add federal and state taxes together when tracking your status. Federal taxes are federal, and state taxes are state.

     

    Please allow eight weeks from the date of submission before calling the IRS for any tracking questions. For any direct assistance, please email thetaxladyllc@aol.com.

We can only use your tax return information for the purpose of preparing and filing your tax return, unless authorized by law or unless you give us your consent.     3. Connecting you with another professional in another field or profession upon your direct request so that you may gain further information on a certain topic not available or in the realm of expertise of this firm. 

    4. Notification of important tax law changes. 

    5. Notification of changes or information that will impact our engagement, such as a change of firm website address, email address, physical location, or other such contact, biographical or geographical information. 

    6. Other reasonable business purposes including but not limited to: appointment reminders, holiday mailings, birthday greetings, and other informational mailings. 

     

    Please note that The Tax Lady LLC. and its owner, employees, or staff, will not use or disclose your tax information for any purpose other than what is permitted by this consent form.   • At The Tax Lady LLC., we value our relationship with every client. Our goal is to provide you with exceptional service. However, it is important to understand that we are providing a service, not a refund. 

    Please be advised, as a taxpayer, you have the option to pay your tax preparation fees using cash, money order, or directly from your refund*.

    If you choose to have the fees deducted from your refund and your taxes are intercepted, you are still responsible for paying the fees. We urge you to contact our office immediately for payment or payment arrangements. By signing your return, you agree to pay the preparation fees regardless of whether you receive a refund or not. If no payment or payment arrangement is made within 90 days from the scheduled refund date, we will be compelled to put your account into collections. We hope to avoid this situation and encourage you to keep us informed so that we can work with you.
