1. Select type of 2025 ADULT MEMBERSHIP. 

• What is a 2025 ADULT MEMBERSHIP:
  • Must be 18 years or older to join.
  • Must pay club dues,
  • complete form and sign liability waiver before competing in your first tournament.
  • All paid members are eligible to compete for Angler of the Year.

Charter Member Dues:

• Deadline to pay dues is at the April Meeting.
• Open to current Charter Members in good standing.

Social Member Dues:

• Social Members may join the club at any time up until the last tournament of the year.

2. SELECT A JUNIOR MEMBERSHIP FOR CHILD(REN) UNDER 18

A paid Adult Membership and supervision of a parent/legal guardian is required for all children under 18 to join the club. Junior Members have no voting rights in club business and are not eligible for a Charter Membership until maintaining an Adult Social Membership for at least one year.

• Junior Membership:
  • FREE for children under the age of 18. Join any time during the 2025 Season. Competes for Junior Angler of the Year. 2025 Junior Member tournament fees are $10 per angler, per event.
• Junior Social Membership:
  • $50 dues can be paid at any time during the 2025 season. Open to children under 18 who are required to purchase a state fishing license (currently 16 years old in Ohio Competes with Adult Members for Angler of the Year. Must pay 2025 Tournament Fees.

4. REQUIRED JUNIOR MEMBER INFORMATION Please print clearly.

5. By submitting this completed form, you confirm that you are an adult 18 years or older. You confirm that you are the parent or legal guardian of the child(ren) named above. You agree to join the club as a paid member and confirm that you are solely responsible for the direct supervision of your child(ren) during all club functions. You agree to abide by all rules and regulations of the Cincinnati Anglers Club in accordance with local, state and federal laws (go to cincinnatianglers.com for more information You agree to sign the liability release form on the next page prior to fishing in your first tournament of the year.

2025 Parent / Guardian and Under 18 Waiver, Release of Liability, Covenant Not to Sue

In consideration of being allowed to participate in any way in Cincinnati Anglers Club fishing activities, website, or related events and activities, the undersigned agrees to the following:

1. I acknowledge and fully understand that I will be engaging in activities that involve risk of damage to personal property, or serious injury, including permanent disability and death, and severe social and economic losses which might result not only from my own actions, inactions or negligence, but the actions, inactions or negligence of others, as a result of boating or fishing, including but not limited to lakes and waterways, acts of other participants, adverse lake and weather conditions, and I hereby assume complete responsibility and liability for such risks. Further, there may be other risks not known or not reasonably foreseeable at this time.

2. I do hereby release, waive, discharge and covenant not to sue Cincinnati Anglers Club, its directors, agents, sponsors or any other individual in any way associated with Cincinnati Anglers Club, all of which are hereinafter referred to as 'Releasees', from any and all liability to me, my heirs and next of kin for any and all claims, demands, losses or damages on account of injury, including death or damage to property, caused or alleged to be caused in whole or in part by the negligence or any other actions of the Releasees or otherwise.

3. I knowingly and freely assume all such risks, both known and unknown, even if arising from the negligence of the releasees or others and assume full responsibility for my participation.

4. I certify that my physical condition, experience and training are adequate to enable me to compete safely in Cincinnati Anglers Club events.

5. I hereby grant full permission to the Cincinnati Anglers Club any and all of the sponsors of this event or their representatives to use any photographs, video, or any other records of this event for any purpose whatever.

6. All prizes may be subject to federal and/or state taxes. Cincinnati Anglers Club assumes no responsibility for winners' failure to report awarded prizes to the appropriate taxing authorities.

7. This waiver may not be modified in any way. If any part of this waiver is determined to be invalid by law, all other parts of this waiver shall remain valid and enforceable.

8. I hereby warrant and represent that I am at least 18 years of age and I have signed this WAIVER, RELEASE OF LIABILITY AND COVENANT NOT TO SUE freely and voluntarily.

9. I hereby warrant and represent that I am the parent or legal guardian for the named junior member(s) below, and that I have signed this WAIVER, RELEASE OF LIABILITY AND COVENANT NOT TO SUE freely and voluntarily in my capacity as their parent or legal guardian on their behalf.

 

I have read the above terms, waiver, release of liability, and covenant not to sue, and understand that I have given up substantial rights by signing it. I have also signed this release of liability, and covenant not to sue freely and voluntarily. NOTE: This form must be completed and returned to Cincinnati Anglers Club prior to all applicant's first tournament.
