I give my permission for my child to particpate in the Lancaster Kiwanis Little League Baseball program. As the parent or legal guardian of the above child, I will not hold the Kiwanis Club, volunteers, or the Lancaster City Schools Board of Education liable for any accident that may occur while he/she is playing or practicing on the grounds of the Little League baseball fields located at Tallmadge Elementary School or during any other Kiwanis league activity held at other locations. I also hearby pledge to provide positive support, care, and encouragement for my child and all the children, coaches, officials and other parents involved with the Kiwanis Little League. I have read and agree with the Family Code of Conduct.
