VALID US Passports are required for travel to the Dominican Republic. The US Department of Homeland Security and TSA requires your full given Legal Name, First, Middle and Last as well as Date of Birth, Citizenship and Gender. Failure to provide this information fully and correctly may result in costly change fees if it later has to be corrected or you may be denied boarding at the airport or entry into the Dominican Republic. Passports should be valid for six (6) months after your travel date. Please check to make sure your registration matches your identification document or Passport exactly.

If your traveling companion is paying separately and has a different address and phone number, please submit separate registration forms and indicate "traveling with" and name for PAX 2.
A deposit of $210.00 per person is required at the time of registration. A second deposit of $500 per person is due on or before April 20, 2024 and final payment is due on or before July 8, 2024.
