This form is not a commitment and no payment will be requested at this time. Upon completion I will contact you as soon as I am able to answer any questions you may still have, or to discuss potential dates and times for your appointments.

I enjoy being a passionate tattooer, but even more than that I enjoy being a husband and father of four. I love to stay busy in and out of work and because of that it may take a few days to respond. Hang in there! I haven't forgotten about you. Our youngest has probably just spilled kool-aid on the carpet again.

I will take anything into consideration, so don’t be shy. I work with one client a day to allow me to give you my undivided time and attention while we're in the studio together, and I maintain a two hour minimum per appointment, regardless of size.

Please read the questions and answer to the best of your ability. Believe me, I know how hard it is to describe an idea, design, or vision sometimes. That being said, try your best! The more information I get, the quicker and easier it is for me to book you.

Please upload a minimum of 1 photo, but up to 10 photos. This should include include reference and inspiration pieces, or a photo, if appropriate, of where you'd like the tattoo on your body. For specific images please upload the image.

PRIORITIES: I have a 2hr minimum per appointment.

This does not mean I don’t accept smaller tattoo requests! But it does mean that if your tattoo only takes 45 minutes, you will still be billed for 2hrs. If you’re looking at pieces that don’t take much time, for your benefit, I recommend finding a few smaller tattoos to do at once instead of on separate occasions.

NO minors. Sorry, the city our shop is located within does not allow tattoos to be performed on anyone under the age of 18.

ALL consultations are done through text or e-mail following the completion of this form. (Text preferred)

NO designs will be sent out prior to your appointment.

Unfortunately, due to the amount of time invested in a design, I am unable to provide you with an image before your first visit. Most drawings are done when you arrive to ensure it properly fits your body, and so that I know we’re both on the same page as far as size and style are concerned. We will be able to talk, via email, about your piece leading up to your appointment. I also allocate extra time during your visit to review the design we develope, and so that we are able to make any changes before we begin. I do not charge for this time.

Using “small, medium, large, etc.” as a description doesn’t give me much to go on. If you’re unsure of the dimensions try to use common objects as a reference. (business card, sticky note, postcard, full sheet of paper, etc.)

Placement(s) on body.*

Please keep in mind you are paying for a one of a kind piece of artwork that will be with you for life, my pricing also covers my supplies.

My hourly rate is $150/hr, with a 2hr minimum per appointment. I only bill for time spent tattooing you. (I do not charge for draw time, design, stencil placement, setup, etc.,)

YOU DO NOT NEED TO SEND ME ANY MONEY AT THIS TIME. I will follow up with you in regards to a deposit when it is time to pick your appointment date(s).

Please note that a non-refundable deposit of $100 per appointment is required in order for you to secure your spot(s). The deposit goes towards the price of the tattoo. Your deposit MUST be made in order to secure your time slot(s), once I respond to your request and we decide to move forward with your tattoo, you will receive a link which gives instructions on how to make a deposit.

Do not send me money if I have not requested you to do so. Any money sent to me without a specific request from me will be considered a donation.

ATTENTION NEW CLIENTS!

If an existing client of mine referred you, both yourself and the person who referred you to me will receive $50 in credit to use any time you'd like on a future appointment when you fill out their first and last name below!

Referral Program Fine Print

Credits will be awarded to both customers if and when the new client's first appointment is booked. Credits cannot be used for deposits. Credits can only be redeemed when paying for services. Credits can never be withdrawn as cash, transferred to a card or another account, or exchanged in any other way. Loss of your appointments or eligibility as a client due to no shows, cancellations, or for any other reason will result in the forfeiture of any unused credits.

You are only a new client once. If I've ever tattooed you, or you've already booked appointments, you are an existing client. Once I've tattooed you, or anytime after you've booked your first appointment, you can refer as many new clients as you'd like and stack those credits up!

1. For the most part, I will be replying to inquiries in the order in which they were received. Please ensure you have answered all relevant questions on the form.

2. Please reply to my texts as promptly as you can - my initial reply to your inquiry may take a few days, sometimes longer, but once I reply I am relatively quick in responding to questions and requests to get your appointments set up.

3. My books are always open but occasionally I don't always take someone on as a client. This is nothing personal and there could be a number of factors that go into this decision (such as concept, placement, timing, my own level of experience with your desired tattoo idea/placement, etc). But if I can't tattoo you I will happily recommend you to, or help put you in touch with, an artist I trust for your process.
