Football Camp

PTP Sports hereinafter shall be referred to as "State Organization" and its (their) directors, officers, employees, coaches, agents, representatives, independent contractors, subcontractors, suppliers, sponsors, successors and assigns ( all of whom are hereafter referred as "The Releases")

Clinics, Camps, Tryouts, 7V7 Team, 5V5 Team, and Tournaments hereinafter shall be called the "Activity".

 Definition

In this Release Agreement, the term “Activity” shall include all activities, events or services provided, arranged, organized, conducted, sponsored or authorized by the Releasees and shall include, but is not limited to: “Activity”; “Activity” rental; orientational and instructional courses, seminars and sessions; travel, transport and accommodation; and other such activities, events and services in any way connected with or related to “Activity”.

 I, parent and or guardian do hereby waive, release discharge and covenant not to sue "Stated Organization" from any and all liability from any and all claims including negligence of the "Stated Organization" resulting in personal injuries, accidents and or illnesses including death and property loss arising from, but not limited to the participant's participation in the "Activity"  I also fully release the site, site staff, city of site and any and all other participating entities in the course of participating in the "Activity"

I agree to hold harmless and indemnify "The Releasees" for any and all liability for any and all claims including negligence, property damage, loss or personal injury to any third party resulting from any and all claims, actions, suits, procedures, costs, expenses, damages and liabilities, including attorney's fees or to reimburse them for any such expenses incurredbrought as a result of the participant's involvement in the "Activity".

 This Release Agreement shall be effective and binding upon my heirs, next of kin, executors, administrators, assigns and representatives, in the event of my death or incapacity;

 This Release Agreement and any rights, duties and obligations as between the parties to this Release Agreement shall be governed by and interpreted solely in accordance with the laws of the province where the "Activity” takes place and no other jurisdiction; and

Any litigation involving the parties to this Release Agreement shall be brought solely within the province where the “Activity” takes place and shall be within the exclusive jurisdiction of the Courts of that province.

I parent and or guardian am aware that the “Activity” involves many risks, dangers and hazards. The risks, dangers and hazards, including but not limited to: loss of balance; difficulty or inability to control one’s speed and direction; variation or steepness in terrain; rapid or uncontrolled acceleration on hills and inclines; mechanical failure of equipment; variation or changes in the playing surface including rocks, gravel; changing weather conditions; exposure to temperature extremes or inclement weather; travel or transport to and from the sites used for “SPORT”; travel on highways and backcountry roads; encounters with domestic and wild animals including dogs, bears and cougars; collision with pedestrians, motor vehicles, cyclists and other players; failing to play safely or within the limitations of one’s own abilities, negligence of other participants; and NEGLIGENCE ON THE PART OF THE RELEASEES, INCLUDING THE FAILURE ON THE PART OF THE RISKS "RELEASEES" TO SAFEGUARD OR PROTECT ME FROM THE RISKS, DANGERS AND HAZARDS OF THE “ACTIVITY”.   I AM AWARE OF THE RISKS, DANGERS AND HAZARDS ASSOCIATED WITH THE “ACTIVITY” AND I FREELY ACCEPT AND FULLY ASSUME ALL SUCH RISKS, DANGERS AND HAZARDS AND THE POSSIBILITY OF PERSONAL INJURY, DEATH, PROPERTY DAMAGE OR LOSS RESULTING THEREFROM.

Any participant who does not abide by the rules, regulations and policies established by PTP Sports is subject to dismissal without reimbursement or recourse.

I myself the parent/guardian authorize consent for the participant to attend the "Activity".  In the event of any injury or illness I hereby give full consent for medical treatment.  Furthermore, I myself as parent/guardian agree to be responsible for any charges or fees resulting from the participant's attendance and participation at the "Activity".  I acknowledge that I fully and completely understand the potential risks that my be associated with this activity and that my child's participation is strictly voluntary

I hereby authorize "Stated Organization" and its "Releasees" the right to take photographs or videos of myself and or registered participant a minor child our names and likenesses, for the use in the "Stated Organization" print, online and video based marketing materials, as well as other "Stated Organizations" publications.

I hereby release and hold harmless the "Stated Organization" from any reasonable expectation of privacy and or confidentiality and or images associated with myself and for the registered participant a minor child.

I hereby release the "Stated Organization" and  "Releasees" involved in the creation or publication of the "Stated Organization" publications, from liability for any claims by me or any third party in connection with my participation or the participation of the registered participant a minor child.

I have read all previous paragraphs, including the Waiver Of Liability, Indemnification and Hold Harmless, Severability, Assumptions of Risk, Behavior Guidelines, Medical Authorization, Photo/Media/Publication Authorization Release know, fully understand its terms, acknowledge these and other risks that are inherent to the "Activity" and understand that I am giving up substantial rights, including my right to sue.  I acknowledge my child the participant's participation is voluntary, that I knowingly assume all such risks, and that I am signing the agreement freely and voluntarily, and intend by signature to be a complete and unconditional release of all liability to the extent allowed by the law.
