IYNZ Level 1 Assessment Application

BEFORE YOU BEGIN FILLING IN THIS FORM, PLEASE ENSURE YOU HAVE THE REQUIRED DOCUMENTS COMPLETED & READY FOR UPLOADING AS PART OF YOUR APPLICATION:

1) Referral Form
Download the Referral Form via this link and ask your Teacher Trainer to complete the form. You or your Teacher Trainer can scan or photograph your completed form for uploading (Refer Section 5c)

2) CPR Certificate
Your CPR Certificate must be current as at the date of your assessment. (Refer Section 8)

3) Description of Your Home Practice
In a separate document, please describe in detail your home practice. Include how often you practise, for how long and the asana and pranayama you include. Ensure your name clearly indicated on the page.
Note: This should be maximum 1x A4 page and approximate font size 10 or 12.
(Refer Section 9)

4) Proof of Assessment Fee Payment
The Level 1 Assessment fee is NZD$350.
Online payment should be made to IYNZ: 06-0199-0081273-00. Reference: ‘Assessment/Name’.
Your Proof of Payment can be a photograph or screenshot of your banking records showing the deposit, or a bank deposit slip. (Refer Section 13)
