Pet Memorial Form Submission

Honor your beloved pet with a heartfelt tribute. Simply fill in the form below, upload one photo, and we will craft and display your pet's tribute pet on our website for one month.

Memorials will be posted weekly on Sundays. The deadline to submit a tribute form will be the Thursday prior to Sunday posting. A nominal fee of $5.00, to benefit the shelter pets at the Erie Humane Society, wil be applied to display your tribute.
