I confirm that I am physically and psychologically fit in participating in this training, and acknowledge and agree to the following items, on behalf of myself and my heirs, guardians, representatives, successors and assigns. I fully understand the risks involves in this training activity. I understand that this is serious and life-threatening. I understand that the use of Firearms may involve risks (unforeseen or otherwise) and that I am responsible for my own safety and behavior, including but not limited to the use of appropriate personal protective equipment and refraining from the use of substances that may alter my mental state. Tamara Dumont does not provide guidance on the specific legality of skills learned at these events. I understand that I am responsible for determining the requirements of all applicable laws as they may relate to ownership, possession and use of firearms, and remaining in compliance with such requirements. I release Tamara Dumont from any liabilities like injuries, damages, accidents, or death. I release, waive, and indemnify Tamara Dumont, from any accidents, injuries, damages, or death during the training classes. I confirm that I am 21 years and above and I am legally allowed to own and use a firearm. If Tamara Dumont believes that I am involved in any suspicious or criminal activities, Tamara has the right to expel me without a refund. I confirmed that all information entered in this form is true and accurate.
