PLEASE COMPLETE WITH PARENT & TEEN: All teens driving to Wisdom Builders need to read this form and sign it at the bottom. All parents with teens driving to and from Wisdom Builders need to read this form, fill in their Teen Driver’s privileges, and list any siblings that may be transported by the Teen Driver. Once completed, sign it at the bottom. Upon completing this form, you will receive a parking pass to be placed on your teen driver's vehicle's dashboard, which will be placed in your family folder.

Guidelines | All Teen Drivers MUST:

Ø    have a valid driver's license. (No Driver Permits allowed.)

Ø    adhere to a 10 MPH speed limit in the parking lot. Many young children walk with their parents to the church building, and sometimes they run to the door and, due to being small, are not easily seen. Teen Drivers are to drive VERY slowly and CAUTIOUSLY in the Wisdom Builders / Grace Church parking lot.

Ø    park in the designated parking spaces at Grace Church.

Ø    NOT drive other non-family members' children or teens in their car.

Ø NOT leave campus for lunch

Ø   arrive at Wisdom Builders in time for programs and activities but not earlier than 10 minutes before their first class at Wisdom Builders. Moreover, when their programs and activities are done for the day, Teen Driver should leave Wisdom Builders promptly and not "hang out" without consulting the director. No teen driver may leave the premises with any passengers not specified in this agreement.
