    Middle Park Medical Foundation Scholarship Guidelines and Application

  • Scope:

    The Middle Park Medical Foundation (MPMF) provides scholarships to eligible employees of Middle Park Health (MPH) who have gained admission to colleges, universities, vocational institutions, or continuing education/training programs in healthcare sciences. Scholarship selection is primarily determined by factors such as an applicant's career objectives, their employment history at MPH, their involvement in the community, and their financial requirements. Upon confirmation of enrollment, the scholarship funds will be disbursed directly to the educational institution chosen by the recipient. Once the MPMF annual scholarship budget dollars are exhausted for the year, no further scholarships will be awarded until the next fiscal year, which is January of the following year. All applicants are encouraged to reapply at that time.
  • Application Eligibility Requirements:

  • 1. Before applying for an MPMF scholarship, applicants are required to utilize all Continuing Medical Education (CME) funds, if applicable, provided by MPH.


    2. MPH employed physicians are not eligible to apply for MPMF scholarships.


    3. To be eligible, applicants must have completed a minimum of 1 year of service at MPH and should have the capacity to continue their work at MPH.


    4. Applicants must be currently employed at MPH and work a minimum of 30 hours per week.


    5. Applicants are required to provide two letters of recommendation, excluding any from family members. These letters should come from faculty, employers, or other community members who are familiar with the applicant's personal qualities and work habits.


    6. Applicants must submit their most recent performance evaluation received from MPH.


    7. Applicants should not be under a Performance Improvement Plan (PIP) at the time of application.


    8. All sections of the application must be completed in full. Please note that incomplete applications will not be considered.

  • Scholarship Awards and Service Commitment Guidelines:

  • 1. Employees who have served at MPH for a minimum of 1 to 3 years are eligible to apply for a scholarship, with a maximum limit of $3,000 per year. If awarded, the scholarship recipient must commit to serving MPH for a duration of 1 year. This service commitment period commences from the date when MPM Foundation disburses payment to your educational institution.


    2. Employees who have dedicated a minimum of 3 or more years of service at MPH can apply for a scholarship, with a maximum limit of $6,000 per year. Scholarship recipients in this category are required to commit to serving MPH for a period of 2 years. The service commitment period begins from the date when MPM Foundation makes the payment to your educational institution.


    3. If the employee fails to meet the service commitment years, as defined above, the employee will reimburse MPH for the prorated share of the funds paid to the educational institution, before his or her last day of employment at MPH.
