  • Questionnaire for Income Tax Year 2025

    Please fill the questions and provide all necessary information along with supporting documentation. We will review and give you call for any questions or missing documents. As always feel free to stop by our office to fill the questionnaire in person and to also speak to us for any tax related matter. If at this time you are missing tax related support documentation, you can complete the questionnaire and you can always email the documents, fax them or drop off at our office. Office hours (Tax Season) Monday - Friday 9:00-7:00 PM and Saturdays 9:00-5:00 PM.

    •       Did you receive payments for goods or services through third-party payment platforms (such as Zelle, Venmo, PayPal, Cash App) totaling more than $2,500 during the 2025 tax year?

      If you received 1099K from a business account, please explain the job you 
      performed/professional services provided on the next page and bring the related expenses for this activity. Personal transfers such as gifts or reimbursements are not taxable and should not be included

      To qualify, you must:
      • Provide the original purchase documentation from the dealer showing purchase details.
      • Vehicle is for personal use, not for resale.
      • Use the vehicle primarily within the United States.

    •       Do you have any interest income (such as from savings accounts, CDs, bonds) or dividend income (such as from stocks, mutual funds, or other investments) for the 2025 tax year?
      If yes, please provide copies of the following forms: Form 1099-INT (Interest Income), Form 1099-DIV (Dividend Income), Any related brokerage or financial institution statements

    •       Did you make any energy-efficient improvements to your principal residence in 2025 (such as installing insulation, energy-efficient windows, doors, heat pumps). If yes, please provide support documentation (type of improvement, date of installation, cost including labor, manufactures certification)

    •       Was your divorce or separation agreement finalized on or before December 31, 2018, and did you pay alimony under that agreement?
