  • Experience and education in pediatric diabetes. Please briefly describe your experience and training in pediatric diabetes care, including:

    • Number of years of direct clinical experience in the care of children and adolescents with diabetes
    • Current role and setting (e.g. clinical care, training program)
    • Any formal education or training in pediatric diabetes (if applicable)
    • Please specify whether your experience is exclusively pediatric, adult, or mixed, and indicate the approximate proportion of pediatric activity

  • Additional pediatric diabetes-related activities. Please describe any additional pediatric diabetes-related activities beyond routine clinical care. In particular, please specify if you have:

    • Led or participated in quality improvement initiatives (especially related to educational or clinical activities for people with diabetes)
    • Participated in a diabetes educator course and/or ISPAD Science School or ESPE DOM School and/or other structured additional education in pediatric diabetes
    • Held leadership roles or demonstrated leadership potential
    • Contributed to ISPAD activities (e.g. committee work, volunteering, JENIOUS leadership or ambassador roles, speaking at ISPAD congresses in the last 5 years)

    • A structured outline of the planned activities
    • The expected outcomes, identification of existing gaps in the applicant’s home centre/region/country, and a clear, action-oriented plan for how the acquired skills and knowledge will be implemented to address these gaps, including the anticipated impact.
