This form will help us learn more about your organization and the pet you have rescued before their stay with us. Please fill it out completely and Honestly, we ask some of these questions to ensure the pets safety, and the safety of our staff during their stay with us.
Our Policies for Boarding:
-ALL pets must be up to date on their core vaccines (or atleast their first round of boosters) PRIOR to drop off.
-Core Vaccines required are Parvo/Distemper, Rabies, and Bordetella.
-We do also require a Leptospirosis vaccine, and so do many northern states.
-If the pet is coming in without having received a negative fecal exam, they are required to have atleast 1 round of dewormer administered prior to boarding. If there are any irregularities with the pets fecal during boarding we will require a fecal test with your vet (or ours) and the dog will be placed under quarantine to ensure we limit parasite cross contamination.
-Our Rescue Boarding rate is $20 per day, per dog. Mom's with 1-4 week old puppies is also $20 a day, but once the puppies reach 5 weeks old the rate will increase to $30 per day until they are 6-7 weeks old. When mom decides they can't live in her space anymore, they will be moved to their own kennel which increases the rate to $40 per day for the mom and her puppies to be in separate kennels. Puppies that come in without their mom will be charged $40 per day as a group, and can not share a kennel past 16 weeks of age. Unless otherwise specified in writing by our Director.
-We will provide the pets with all the food and treats they will need during their stay, but if you would like them to be on a specific diet- your organization must provide those food/treats entirely.
-We must receive your pets medical records PRIOR to your dogs stay with us via email, text, or Facebook messenger. You or your vet can email your pets records to us at DogsGoneWildBoardingInfo@gmail.com
-The pet's medical records will be reviewed by our Director and HAVE to be approved prior to drop-off of the pet. Please send them to us IMMEDIATELY after sending us this boarding request form. You may email, text, or send them via Facebook messenger.
-Drop off and Pick up times are "Set in stone", please do not be late, if you or your transporter/foster are more than 30 minutes late there will be a $20 late fee charge within reasonable circumstances. If you know you are running late, please let us know so we can adjust our schedules.
-Payment will be due weekly for rescues and will be due upon drop-off for any public boarding clients. ALL unpaid balances will be due prior to pick-up of the rescue pet. We accept Paypal, Venmo, Cashapp, or Credit/Debit card. All payment discussions and agreements need to be in writing via email, texts, or messages to ensure we are all on the same page. Late payments may incur a late fee of $5 per day after billing is sent out. Billing will be sent out on Fridays, unless otherwise agreed upon in writing with our Director.
-If your organization defaults on payment for two weeks past an invoicing date, Dogs Gone Wild Reserves the right to consider that pet abandoned in our care and therefore, at that point, your organization will be forfeiting ownership rights. Ownership will automatically be transferred to Dogs Gone Wild, and the dog will either be returned to the shelter it originated from (which may result in tagging rights for your organization being revoked for abandonment) or Dogs gone Wild staff may release the care of the pet into a different 501c3 organization.
-If your dog shows signs of illness (Non-emergency) he/she will be secluded from other pets for the remainder of their stay and we will let you know promptly what symptoms they are having.
-If your dog shows signs of aggression towards staff that can not be worked with safely, we may ask that he or she be picked up immediatly to ensure our staffs safety.
- If your dog shows signs of aggression towards other pets, they will be secluded from interacting with them during their stay, but in most cases can still happily and safely enjoy their stay with us.
-If the pet is destructive they may be moved out of the large kennels and placed into an "Impact" style crate to ensure they do not harm themselves or cause further damage to the facility. We will inform you of this decision immediately. The pet will still be given water in the crate at all times and will be fed in their crate as well, but will be taken out into play areas and walked on a leash for the remainder of their stay.
-Early Pick-ups will still be charged the full amount for that day.
-If the pet is displaying medical concerns that are life threatening due to a preexisting medical condition, we may request that a representative from your organization picks them up immediately (in this case boarding costs will be billed on that weeks Friday per usual, but the pet can still leave with your organizations representative immediately given the circumstances.) If there is not a representative available to pick up the pet, then we will be forced to bring them to an emergency hospital and you will be billed for the visit immediately after and prior to picking up the pet from our care. If a representative from your organization is not able to communicate with us during the pets emergency vet visit to make decisions on the pets care, then we will make the medical decisions for the pet based on our 10years of experience in rescue with great consideration and compassion for the pet's quality of life and longterm health.
By signing and submitting this form, you agree to these policies and understand that we reserve the right to refuse the boarding of any animal that shows signs of illness or aggression at the time of drop-off and that we may ask you to pick the pet up if illness or aggression becomes present while the pet is in our care for the safety of our staff and other pets in our care. We do also house Rescue dogs on the property for several other organizations, by signing below you understand and agree not to hold Dogs Gone Wild liable for any cross contamination of illness/disease. We do everything we can to prevent the spread of contagious pathogens, including requiring all of the core vaccines and following strict quarantine guidelines- but in any boarding scenario, some level of risk of cross contamination is always present.
