I, the undersigned parent or legal guardian of the minor participant named below, hereby acknowledge and agree to the following terms in connection with my child's participation in the scout activity listed above:

1. Assumption of Risk: I understand that participation in the Activity is voluntary, and involves inherent risk during participation, including the risk of possible accidents, physical injury, or exposure to the COVID-19 virus or other infections or infectious diseases as a result of attending events, meetings and activities.
2. Health and Medical Information: I confirm that my child is in good health and is physically capable of participating in the activity. I agree to inform the activity leaders of any relevant medical conditions, allergies, or medications affecting my child's participation.
3. Emergency Medical Treatment: In the event of an emergency, I authorize the leaders of the activity to seek necessary medical treatment for my child. I also acknowledge that I am responsible for any medical expenses incurred.
4. Behavior and Conduct: I agree that my child will adhere to the Plast code of conduct and follow the instructions of the activity leaders. I understand that any violation of rules may result in my child's immediate dismissal from the activity.
5. Photography and Publicity: I grant permission for photographs, video recordings, and other media to be taken during the activity and used for promotional and other purposes. Please be advised that buses and other forms of public transportation may use video surveillance equipment.
6. Waiver and Release: I hereby release and forever discharge Plast Ukrainian Youth Association Kitchener-Waterloo, its directors, officers, agents,
   servants, other participants and employees and its or their successors, heirs, and assigns (the “Releasees”) should any injury or loss of limb or life happen to my person/to my child whilst partaking in "Activity". I also waive claim
   against the “Releasees” should any damage or loss occur against my personal
   property/property of my child whilst partaking in "Activity" and any loss
   of any nature caused by delays resulting from inclement weather or other natural conditions during "Activity" duration and/or participation of the undersigned/my child in "Activity" and notwithstanding that the same may have been contributed to or occasioned by the negligence of the “Releasees” or any of them.
