Liability Release Form for Minor Participants in Outdoor Events and Fly Fishing

I, {name}, as the parent or legal guardian of {name107} hereby give permission for the above referenced child to participate in outdoor events and fly fishing activities organized by Girls on the Fly. I understand that these activities involve certain risks and dangers, and I agree to release Girls on the Fly, its directors, officers, agents, and volunteers from any and all liability for personal injury. property damage, or wrongful death arising from my child's participation in these activities. I acknowledge that my child will be participating in activities that take place in outdoor environments, including but not limited to rivers, lakes, streams, forests, and mountains. I understand that there are risks associated with being outdoors, such as exposure to weather conditions, insects, and wildlife. I acknowledge that these risks may be increased due to the activities of fly fishing, which include wading in water, using fishing equipment, and handling fish. I understand that my child will be supervised by trained instructors and volunteers, but that accidents can happen despite their best efforts. I agree to release Girls on the Fly, its directors, officers, agents, and volunteers from any and all liability for any injury, loss, or damage suffered by my child during the course of these activities. I agree to assume all risks associated with my child's participation in these activities, including but not limited to the risk of personal injury, property damage, or wrongful death. I certify that my child is physically able to participate in these activities and that they do not have any medical conditions that would prevent them from doing SO. I hereby waive and release any and all claims that I, my child, or any other person may have against Girls on the Fly, its directors, officers, agents, and volunteers arising from my child's participation in these activities. I understand that this waiver and release is intended to be as broad and inclusive as permitted by law, and that if any portion of it is held invalid, the remainder will continue in full legal force and effect. By signing below, I acknowledge that I have read and understand this Liability Release Form and that I am giving up substantial rights by signing it. I agree to it voluntarily and without inducement or coercion.

I, {name} hereby authorize Girls on the Fly, hereafter referred to as "Company," to publish photographs taken of the above listed participant during event dates: June 15, July 20, August 17 or September 21, 2024 and their name and likeness, for use in Girls on the Fly print, online and video-based marketing materials, as well as other Company publications. I hereby release and hold harmless Girls on the Fly from any reasonable expectation of privacy or confidentiality associated with the images specified above. I further acknowledge that participation is voluntary and that participants will not receive financial compensation of any type associated with the taking or publication of these photographs or participation in company marketing materials or other Company publications. I acknowledge and agree that publication of said photos confers no rights of ownership or royalties whatsoever. I hereby release Girls on the Fly, its contractors, its employees, and any third parties involved in the creation or publication of marketing materials, from liability for any claims by me or any third party in connection with my participation.

*We will take photos of all the girls (hopefully with fish) and send to the guardians after each event. Those who opt out will receive the photo to the email associated with the participant and no other photos will be shared or distributed.
