• First Line Weatherization Intake Form

  • This form is NOT the full application. Filling out this form does NOT guarentee that your household will be eligible to recieve free home weatherization through Project Warm.  

    Thank you for your interest in Project Warm's First Line Weatherization Program! Through this program, you may be eligible to recieve various home weatherization repairs such as: replacement of cracked or broken window panes, air sealing around doors and windows, plastic over windows, new door locks, and other minor weatherization repairs.

    To be eligible for our FREE First Line Weatherization services, your household must meet all of the following criteria:

    • Be located in one of the following counties: Jefferson, Bullitt, Oldham, Shelby, or Spencer
    • Recieve utility service(s) from LG&E
    • Have a total household income that does not excede 200% of the yearly federal poverty guidelines (2024)
    • Have not recieved our First Line Weatherization services in the last 3 years

    Once your intake is submitted, a Project Warm staff member will confirm your eligibility status within 3 buisness days. At this time, a Project Warm staff member will also provide the full application to those who qualify.

    If you are having difficulties completing the intake online, a Project Warm staff member can assist you in completing it over the phone at (502) 636-9276. Office hours are 8:30AM to 5PM, Monday through Friday.

  • If you are are a Project Warm employee AND the intake is being preformed by someone who is not the Applicant, fill out the questions below. If the Applicant is performing the intake themselves, you can continue on to the next page.
