This allows me to get to know you and your hair goals so that I can ensure your initial appointment is booked properly. * Please complete the form in full and either go back to the website to book or I’ll be in touch within 1-2 days of your request during my work week which is Wednesday-Saturday. * All requests submitted Sunday-Monday will be responded to on Tuesday. By completing this form I understand :

Your appointment in salon will begin with a 15 minute full consultation to refine the plan for service. During this experience you'll discuss your hair history, goals for today and goals for the future. We will also discuss your hair care routine to ensure you are 100% confident with your styling results betwen visits.

I understand that we have reserved this time especially for you and require a minimum of 48 hrs. notice for cancelations or changes to services.

If you need to cancel your appointment within the 48 hrs. you will be charged 50% of the cost of the services. Repeated cancellations may result in being required to place a non refundable deposit before booking.

NO SHOWS: Will be charged 100% of the services scheduled. No Shows may result in being blocked from booking
