T5/SWPT Commitment 2025
  • This agreement is between * (you) , and both T5 Fitness Ltd and SWPT Ltd (us). This agreement is together with the full terms and conditions presents upon joining us.


    This is our commitment to each other. This will ensure the highest level of service from me and my team to you, and in turn the highest level of engagment from you. We do this ensure the best results from working together.


  •  

    • Your agreement with us is made up of your completed and signed Health Screening and this signed document. These documents together form a legal agreement between you and us, so please make sure that you read them carefully and understand them. If you have any questions, please ask a member of our team.
    • This agreement form part of any agreement you have with T5 Fitness Ltd and/or SWPT Ltd

     

    Commitment for 1-1 sessions:

     

    • All sessions/rearranged sessions MUST be completed within 3 weeks of which they have been booked for. There will be no ‘carry over’ for unused sessions after the 3 weeks, unless extreme circumstances are allow by the trainer. The 24 hour cancellation rule still applies. If you cancel and there is another space available, we will offer it. But be aware there might not be.

     

    • Your Trainer is entitled to up to 4 weeks holiday per year. When we go on holiday, you won't have a session (obviously). But you will be provided with a clear plan of action to maintain the momentum of your health and fitness journey. This will be arranged at least one month in advance before we take leave.

     

    • If you take a holiday then you must rearrange the sessions you would miss within 3 weeks of the original booking. You must inform us when you plan and book your holidays so we can make alternative plans for your programme. Please provide a minimum of one calendar month’s notice so we can rearrange appropriately for you. If you fail to provide notice, we cannot guarantee you programme will be set apporiately

     

    • Coaching is based on a minimum term of 1 month. If you wish to cancel at the end of your initial period, then you'll need to give us 30 days notice before the period end date. After that initial period, you need to provide us with at least 30 days notice of any changes or cancellations. If your notice is within 10 days of your payment date, you final payment will be taken in full. (If you cancel on and including 1st-10th January, and your payment date is 11th January, then your full payment due on the 11th January will be taken). If your notice is given more than 10 days of your payment date, then a prorata fee will be charged for the reminder of the notice. The final prorata fee will be taken immediately upon notice.

     

    • You must commit to the same time every week and rearrange if and when we need to make sure you abide by the cancellation and rescheduling rules.
      • Unless you work irregular shift patterns, for which at least 4 weeks of sessions must be booked in advance. If you forget to book a session in a week, you will lose that session. As a shift worker, you may, with the trainer's permission book a session extra in a week, if you are enable to attend a weekly session due to work. However this must be agreed at least 2 weeks in advance with your trainer.

     

  • Commitment for Small Group PT sessions: 

    • All "Team" Small Group Sessions are unable to be arranged. If you are unable to attend a session you will not be entitled for refund or re-arrangement.
    • If you give 7 days notice of not be able to attend a session and there is another space available the same week of the missed session, we will attempt offer it. But be aware, this is not guaranteed.

    • There will be 3 weeks per year where there is not a session (including Christmas and Summer Break). But you will be provided with a clear plan of action to maintain the momentum of your health and fitness journey. This will be arranged at least one month in advance before we take leave.

    • If you take a holiday, then missed sessions will not be replaced, unless agreed by the trainer in line with previous terms.

    • You must commit to the same time every week. However, you can request to change your regular time, if space is available. This must be agreed at least 2 weeks in advance with your trainer and is not guaranteed. If you do swap time, you may not swap again for at least 3 months

  • Lateness

    • If you are late for your session, your session will be reduced in accordance with that time. If you are more than 20 minutes late, your Personal Trainer may choose to cancel the session and you may lose that session, in which case no refund will be made.
  • Your Trainer

    • Your Trainer will be suitably qualified to help you. We will do all we appropriately can to help you. Our Trainers will continue to update and grow on resources and support through our media and support channels.
    • You Trainer is not medically trained, however. We will help where with can with suitable advice, but we will not take responsiblity for any advice with give. Any medical or specialist issues need to be seen by a Doctor or a qualified specialist. We may stop you attending sessions until we have clearance for your to train by these specialists. If this happens, we will not refund any sessions.
  • Miscellaneous

    • If you relocate, and are unable to reasonably travel to sessions, you will still need to give full notice to change or cancel membership.
    • We may change the terms of this agreement from time to time and will notify you of any changes made.

     

     

    By signing this agreement, you are agreeing to do your best, and engage with our coaching. Fitness journeys are hard and have ups and downs, and our commitment to you is that will help any way we can.

    You will be commited by following our coaching to the best of your ability, you will respect our conditions and proceedures, and you will actively work towards your agreed goals.

    If you do not show commitment and engagement, we reserve the right to cancel your membership and give no refund.

    We are here to help and guide you, but you will be putting in the work. Then you can achieve some great results!
