A signed Media Consent Form is required for individuals who may be interviewed or agree to have their likeness captured at public HFTco events.

I hereby give permission to the HAMILTON FESTIVAL THEATRE COMPANY (the “Company”) and its representatives to take moving pictures, photographs, tapes, and/or videos of me and/or to make recordings of my voice at the Company’s events and events in which the Company participates (collectively “the Company’s Events”).

I, the undersigned, hereby consent to the use of my photograph or likeness in any publication, videotape, pamphlet or promotion by Hamilton Festival Theatre Company (HFTco) or other agencies which are promoting the event produced by Hamilton Festival Theatre Company that I was present at, and promotions related to these productions. By signing this Consent, I understand I am releasing HFTco from any and all liability that may occur as a direct or indirect result of my photograph, the release of my identity, or the public relations materials.

I hereby waive any right that I may have to inspect and/or approve the finished product, any products, or any editorial, advertising, or printed copy or soundtrack or web images that may be used in connection therewith and any right that I may have to control the use to which said product, products, copy and/or soundtrack may be applied. I further waive all moral rights. I also waive any right to royalties or other compensation arising from or related to the use of the images, recordings, or materials used within the next five years.

I acknowledge having read, and understood and agree to the above consent, release, waiver and indemnity and understand that it is binding on me, my heirs, executors, administrators and assigns.
