Waiver
I hereby release Jesse Kirby, Elite Martial Arts, it's staff, members, tournament officials and the venue from any liability due to injuries that I may incure as a result if my participation and/or attendance in this event. I understand that sparring is a contact sport and injuries may occur. I agree to abide by the rules and regulations of the event and assume all responsibility for infringement of such rules. I am aware of my personal medical conditions and certify that I am mentally and physically able to compete in the event.
