Herman Tattoo Submission Form

PLEASE READ THE FOLLOWING POLICIES:

• You MUST be 18+ y/o NO EXCEPTIONS. Legal & valid ID must be provided before your procedure started.

• A $100-$200 non-refundable deposit is required each & everytime you book an appointment (unless stated otherwise) OR it goes towards the session or last session if your piece requires multiple appointments to complete.

• If you have to reschedule for any emergency reason, a 72-hour notice is required in order to move your deposit to a new date. You can reschedule twice, but if you fail to attend the upcoming new appointment, it will result in losing the deposit you have put in. In order to rebook another appointment, another new deposit will be required.

Cash or Zelle are my only payment methods accepted.

• Please eat a filling meal & hydrate properly before your appointment time. This will ensure your blood sugar stays consistent throughout your tattoo.
You may also bring a meal and snacks to your appointment.

• Please avoid high consumptions of alcohol &/or caffeine the day prior to appointment.

• I have rights to refuse service to anyone. I don't tattoo anyone that under influence of alcohol or drugs.

• Please contact me if you are going to be 15+ min late to your appointment. I will try my best to acccomodate, however, your appointment is subject to cancellation/reschedule.

• To keep the the amount of people in the studio to a minimum for everyone's comfort, I don't accommodate guests. However, if you have any medical conditions that requires someone to assist you, please let me know beforehand. Thank you for your understanding.

BUDGET

I have $200 minimum for small tattoos,. For continuous bigger work will be charged with half day $800 or full day $1600. If your session takes more than half day session, you will be charged with whatever is fair according to the amount of time we spend on the session. If you have any further questions about pricing, please let me know.
