To be completed by Community Cat Angels (hereafter referred to as CCA) transporters and all individuals accompanying CCA transporters on scheduled transports for the Rescue. This form must be on file with the CCA Secretary before any transporter may participate in a CCA sanctioned transport. Once you have read these terms and agree to each as a volunteer transporter for CCA, sign and date the waiver on each page.

Because most animals handled through CCA are strays, captured ferals, surrendered pets or community cats, detailed health, temperament and behavior background for these animals may not be available or known to CCA at the time of transport. Every attempt is made to assess the animals for transport as well as adoptability, however; despite the rescue organization's best efforts, the animals may have some underlying health or behavioral issues that were not identifiable during their time with CCA. For this reason, individuals transporting animals from this rescue must voluntarily release CCA from any liability resulting from the discovery of health or behavior issues of the transported animal(s All known veterinary records will be made available for your review prior to transport if you so desire and request these documents in advance of the transport. The animals you transport may have been recently spayed/neutered or treated for various animal infirmities or infections, including transmittable diseases. It is your responsibility to handle these animals, and all animals in your care during a transport, in a humane and careful manner so as to not cause further discomfort to the animal, and to avoid injuries or infections to yourself and other transporters. You recognize and accept as your responsibility that all animals which you agree to transport on behalf of CCA should be properly restrained at all times during the transport, using size- appropriate crates or harnesses, so as to prevent distraction or interference to the transporter while driving. You agree to follow these guidelines in each and every transport you perform on behalf of CCA.

Your signature below indicates and attests that you have read and understand this agreement and its terms and conditions. By signing this form, you are forever releasing CCA and all its affiliates from any and all liability arising from the transport, transfer or placement of the animals from CCA, and you are doing so freely, without duress or coercion, and of my own free will. In addition, as a volunteer transporter for the CCA, you agree to ensure that your auto insurance coverage is valid during these transports; that you will adhere to all posted speed regulations and will drive your transport vehicle in a cautious and courteous manner at all times when animals from the transport are in your possession, care and control. You also agree to submit, to your private insurance provider, any claims resulting from the transport or transfer of animals from CCA and excuse CCA from any liability that is not the result of willful and wanton misconduct on the part of the rescue. You also recognize that CCA is a non-profit organization, and therefore is unable to reimburse you for any expenses incurred as a result of your participation in a transport. All expenses for each transport you participate in are your sole responsibility and you agree to hold harmless CCA from any and all responsibility for these expenses, including but not limited to, gasoline costs, oil, tire repairor replacement, interior cleaning, or exterior vehicle damage as a result of a transport of animals from CCA.

As a volunteer transporter for the CCA, you hereby specifically and forever release and hold harmless CCA, and its agents, staff, affiliates, other transporters and foster volunteers, from any and all liability arising from the transport or transfer of animals from CCA. This release of liability includes, but is not limited to, vehicle accidents, bites or scratches from the animals in your transport; the transference of infections or contagious diseases of the animals that CCA is unaware of at the time of transport. You, the transporter, the transporter's family members, heirs and assigns, indemnify and hold harmless CCA for any and all damages suffered or expenses incurred, including any and all legal fees, in defending any necessary legal action, whether for personal injury, death, property damage, breach of contract or any other action, regardless of whether initiated by the transporter or others, arising out or, or in consequence of, the transport, transfer or placement of these animals, and by signing this release of liability, You understand that You are indicating this release is valid and will remain in full force and effect for any and all transports you may participate in, from this date forward, for CCA.
