• 영역별(의료)사회복지사 수련기관 재지정 신청

  • 영역별 사회복지사의 수련에 관한 규정 제2조에 의하여 위와 같이 영역별 사회복지사 수련기관 재지정을 신청하기 위한 신청서 입니다.

    아래 내용을 확인하셔서 정확히 기재 바라며, 허위 내용 기재 시 수련기관에서 취소가 될 수 있습니다.

    신청 정보 입력으로 해당 정보제공자에 대한 개인정보(성명, 연락처, 이메일, 근무경력, 자격번호) 제공에 동의하는 것으로 간주하며, 해당 정보는 수련기관 관리를 위해 보관 관리합니다.

    ※ 신규 신청과 재지정 신청 제출서류 동일

     

    ○ 공통서류

    1) 수련기관 지정신청서 1부 (별지1호 서식)

    2) 사업자등록 또는 고유번호증 택 1부

    3) 수련계획서 (수련과정, 수련방법, 수련내용 포함) 1부 (별지1호 서식)

    4) 영역별 사회복지사업 수행사실 확인서 1부 (별지2호 서식)

     

    ○ 수련기관 확인 서류

    5) 의료 : 의료기관 개설 허가증 또는 신고필증 택 1부

     

    ○ 수련지도자 확인 서류

    6) 영역별 사회복지 실무경험 확인서 (별지3호 서식)

    7) 사회복지사 1급 자격증 사본 또는 자격증명서 택 1부

    8) 민간자격 확인서 1부

    9) 상근직으로 근무하거나 근무했음을 증빙하는 서류 1부

    10) 재직증명서 1부

    11) 수련지도자 자격 증빙 서류 1부

    12) 보수교육 이수 증빙 서류 1부

  • 수련지도자 정보

  • 각 수련기관 신청을 원하는 기관의 수련지도자에 대한 정보를 입력하는 란입니다. 

    수련지도자는 각 수련기관별 1명 이상 등록이 가능하며, 수련지도자의 자격 요건은 사회복지사 1급 자격취득 후 각 영역별(의료) 사회복지사로 근무한 지 5년 이상의 경력을 갖춘 자가 수련지도자로 등록이 가능합니다. 

    영역별 사회복지사 자격정보 및 민간 영역별 사회복지사 정보를 확인 후 수련지도자로 선정이 가능하오니, 경력이 부족할 경우 수련지도자로 등록이 불가함을 안내 드립니다. 

     

    해당 내용을 확인 후 '다음'페이지의 수련지도자에 대한 정보 입력 및 자료 등록과정을 거쳐야 하며, '다음' 페이지를 누르고 정보를 입력하는 것으로 수련지도자의 정보 제공에 동의한 것으로 갈음하겠습니다.
