Middle Park Medical Foundation High School Scholarship Guidelines and Application

Effective January 1, 2026
Scope:

The Middle Park Medical Foundation will award scholarships up to $2500 to selected Grand or Jackson County High School graduating seniors who have been accepted to any college/university or vocational school to further his/her studies in the field of healthcare sciences. The awards will be based on career goals, academic record, extracurricular activities, community involvement, and financial need. Funds for the scholarship will be paid directly to the financial aid office of the institution of the recipient’s choice upon proof of enrollment.
Application Eligibility Requirements:
1. Submit two letters of recommendation (excluding family members). The letters should be from a faculty or other adult member of the community that is familiar with your personal traits and work habits.

2. Submit one copy of your high school transcript

3. All sections of the application must be completed in full. Please note that incomplete applications will not be considered.

4. Complete applications are due no later that April 18th, 2025.

5. Email foundation@middleparkhealth.org with any questions.
