Mace Tattoo Booking Form

Please read before booking:

must be 18 or older!
-I do machine tattoos
-only book one month at a time
-I work in Albuquerque, NM, but I do occasionally travel and do guest spots!
-my minimum rate is $200, final tattoo cost depends on size and placement
-You can find all of my flash on my Instagram! if you're unsure if a piece is available feel free to ask!
-I do custom tattoos but limit the amount per month, flash designs are typically prioritized but if you have a custom idea please fill out the form regardless!
-I require a $40 deposit for flash and customs. This goes towards the final cost of your tattoo. The deposit is non refundable.
-I will reach out to you via email or Instagram to accept your appointment and tell you how to send the deposit. I use Venmo, PayPal, and Zelle for deposits.
-Those who no call no show will not be allowed to book in the future.
-Tips are not required but greatly appreciated!
