My child is a non-swimmer and, though he/she will be attending the Carnival, will not be participating. (Please note if you tick this option, your child WILL NOT be permitted to enter the water at all).My child is not a confident swimmer and can only swim in the 25m pool.My child is a confident swimmer who can swim 50m independently.My child is currently injured or ill and I have notified the Junior School of their absence.
To encourage participation there will be no pre-registration of events this year. Please discuss the events listed in the Program with your child.

Please note - A student who swims in a 50m event may not swim in the 25m event for the same stroke. A student may swim in the 50m events for some strokes (eg freestyle) and the 25m events for other strokes (eg breaststroke). A student may swim in both the 50m and 100m freestyle events if they wish.

Medical Conditions*

The medical information supplied to the College is correct and up to date. In an emergency, I authorise the school to convey my child to hospital by appropriate transport.Existing Medical Conditions: If my child is asthmatic or anaphylactic, I will ensure that they have the required medication with them and that the medication is clearly labelled with the child's name. They are aware of the appropriate use of their medication and that their medication is for their personal use only.
My child will be collected at the conclusion of the Carnival and will not be travelling back to school on the bus. Please name the person collecting your child below.
