This form must be completed for all exemption requests.

Please note: A medical certificate or a letter from a medical specialist may be required to ensure the exemption is approved.

The exemption form must be submitted prior to the event that will be affected.

In the case of the event being a mandatory regatta, the exemption must be received before the close of entries to be considered. ^*
