Addendum to Driver Safety Treatment Program Policy:
Subject: Change in Urine Drug Screen Policy
This addendum serves to inform all participants of an update to the Urine Drug Screen Policy within the Driver Safety Treatment Program at River City Integrative Counseling Services.
1. Change in Drug Screen Procedure:
Effective immediately, the responsibility for obtaining and completing the urine drug screen will shift from the program’s in-house testing to the client. Clients are now required to obtain a 14-panel urine drug screen from Labcorp or an approved lab testing site on a monthly basis or as specified by their individual treatment plan developed by their clinician.
2. Client Responsibility:
It is the client’s responsibility to schedule, complete and report the urine drug screen to River City Integrative Counseling Staff within the set time frame. The client is responsible for all charges associated with the drug screening, including any fees incurred at the lab.
3. Compliance with Treatment Plan:
The frequency of drug screens may vary depending on the treatment plan created by the client’s clinician. Clients must adhere to the testing schedule as directed by their clinician in order to maintain compliance while enrolled into this program.
4. Effective Date of Policy Change:
This updated procedure is effective as of the date the client signs below.
