AICP Certification Exam Accommodation Request Form

This form should be used by members seeking an accommodation in accordance with the Americans with Disabilities Act (ADA). If you are seeking accommodations for English as a Second Language (ESL) consideration, please use this form instead.

When seeking an ADA accommodation, an applicant must upload documentation from their healthcare provider.

To apply for an accommodation:

1. Complete this form, upload the necessary documentation, and submit. APA will contact you with the status of the request within five business days.

2. If your request is granted, register for the exam on the APA website. Do not schedule the exam with Prometric.

3. After you register for the exam, email aicp@planning.org to request your accommodation be added to your Prometric record.

4. After you receive an email from APA confirming your accommmodation was added to your record, schedule your exam with Prometric.

Please upload the appropriate documentation from your healthcare provider (dated within one year of today's date). This report must be on official letterhead and should include credentials of the healthcare provider. Documentation must include each accommodation requested and the reason for the accommodation.*

All requests for exam accommodations are strictly confidential, and documentation submitted in support of the request is used solely for the purpose of evaluating the request. Failure to provide APA with adequate supporting documentation in a timely manner may cause delay in the review process and may impact the applicant's ability to schedule and take the exam.
