North Davidson Veterinary Hospital has adopted the following Policies:

1. A current Driver’s License, NC ID, Military ID or Passport is required for new clients or when a patient is added to an existing account. *

2. Payment in full is expected when services are rendered. Failure to pay will be considered theft of services and legal action will be taken. (Statue 44A-2 states: Animals will be subject to a lien if treated and/or boarded and services are not paid in full when rendered). $150 AFTER HOURS FEE for patients seen Mon. - Fri. after 5:30pm, Sat. after 12:00pm.*

3. NDVH has discontinued the practice of check holds and/or payment arrangements for services. We can recommend organizations that may be able to help with financial assistance. We accept Visa, MasterCard, Discover, American Express and Debit Cards.*

4. All hospitalizations/surgeries will require a minimum deposit of half of the provided estimate at check-in with the balance due at checkout. Some estimates will be required to pay in full before services are rendered.*

4. Client Surgery Liability Release: I understand that the Doctors and Staff of North Davidson Veterinary Hospital will use all reasonable precautions against injury, escape, or death of my pet. I understand that all anesthesia/surgery involves some risk to my pet. I will not hold the doctors or staff liable under any circumstance for the injury/loss/death of my pet. By signing below I affirm that I understand all risk and I have no further questions concerning the care of my pet.*
