I, the undersigned participant, or, as the legal guardian of the participant named above, and if applicable, the owner of the dog named above, hereby acknowledge and agree to the following terms and conditions for participating in dog training sessions offered by In-Line K9:
- Assumption of Risk: I understand that participating in dog training activities involves inherent risks, including but not limited to, the risk of injury to persons or property. I voluntarily assume all risks associated with the participation of myself and/or my dog in the training sessions.
- Health and Safety: I certify that my dog is in good health and that all behaviours including aggression have been disclosed. I agree to notify In-Line K9 of any changes in my dog's health or behavior that may affect its ability to participate safely in the training sessions.
- Conduct and Control: I agree to maintain control of my dog at all times during the training sessions. I understand that In-Line K9 may, at its sole discretion, ask me to remove my dog from the training sessions if required.
- Release of Liability: I, on behalf of myself, my heirs, executors, administrators, and assigns, hereby release, discharge, and hold harmless In-Line K9, its instructors, employees, and representatives from any and all claims, demands, actions, or causes of action, whether in law or equity, arising out of my participation or the participation of my dog in the training sessions.
- Photographic Release: I grant permission for In-Line K9 to use photographs or videos taken during the training sessions for promotional and marketing purposes.
