Please complete this form by Friday, January 10, 2025 by 5pm. Once Team is selected and notified, your entry Payment of $40 will be due on Friday February 28, 2025 by 5pm (No late payments will be accepted). You can submit payment at www.mcfa.org and select the " Make a Payment" button. If you need an invoice for the school to submit a check please contact Kiah at kiah@mcfa.org.
