This form is for all visitors and guests who are requesting to visit any part of the retreat. This form will help us welcome you when you arrive and ensure that you have the information you need.

1. Once you arrive at Camp Garner Creek, please park at the main building (Mitchell Tabernacle), and come in the front doors.
2. Come in and check-in at the Welcome Table to the left as you walk into the foyer.
3. There, you will be greeted by one of our Retreat volunteers. Tell them your name.
4. The volunteer will confirm that you've filled out this request form and then give you a Retreat wristband as well as a name tag. You can access the schedule, map, etc. at that table as well.
