This form is to register your child / children to attend SMS at St Mary's Mays Hill. We require the following details from you:
- Your details as a parent / legal guardian
- Your attending children's details in this form
- 2 emergency contacts for the children (one can be yourself)
- Your consent on Social Media and Liability for your children
The events will be primarily hosted in the SMMH hall, fortnightly on Friday nights, though dates will be advertised online & a WhatsApp announcements group will be created.

**Liability Waiver and Social Media Consent Form**

I, the parent or legal guardian of the participant(s) named above, acknowledge and understand that St Mary’s Antiochian Orthodox Church Incorporated, youth program, St Mary’s Stewards, for youth under 18 and high school students may include activities held both outside and on church grounds. I am aware that these activities may involve physical exertion, sports, and other associated risks.

In consideration of my child's participation, I release St Mary’s Antiochian Orthodox Church Incorporated and associated entities, its staff, and volunteers from any liability for injuries or damages arising during program activities.

Additionally, I grant permission for St Mary’s Antiochian Orthodox Church Incorporated to use photographs and videos featuring my child for promotional and social media purposes. I understand that these materials may be shared on any and all social / print media platforms associated with the church.
