Release of Information

Under FERPA, a school and or Upward Bound may not generally disclose personally identifiable information from a minor student's education record to a third party unless the student's parent hasprovided written consent.
School to UB Data Sharing

By signing this release form, I agree to allow the Adams State University Upward Bound program to obtain my student’s academic records, i.e. progress reports, report cards, official transcripts, attendance records, Individual Education Plans (IEPs), standardized test scores, from my child’s current, past, and future schools that they attend for the purposes of student and program evaluation. I understand that all released records will remain strictly confidential in accordance with federal regulations.

UB to School Data Sharing

Recruitment Day

As part of the application process, students are asked to participate in a in-person or virtual Recruitment Day Event in March. Students and parents are invited to learn more about Upward Bound and have the chance to tour the campus, meet current students, and learn the general requirements of the program. (If you are currently in high school, please select Not possible on all choices.)
