Please read before you train:

Thank you for your interest in training wit Ball Game Athletics ( Reggis Ball) at DPAC ATL.

This is a contract-based service and the following information outlines the information that will be catered to each client based on their needs.

LOCATION:

Reggis Ball with a mailing address of 2040 Headland Drive, East Point, Georgia, 30344

SERVICES. The Personal Trainer is obligated to provide training that enables and instructs the Client to personal betterment regarding their body. That includes strength training, weight lifting, cardio, HIIT training, and more. The Client can dictate the training methods that should be followed to accomplish their goals

DURATION. The Services provided by the Personal Trainer will last for a period of 60 minutes per training session (“Session”). Clients are expected to attend each session on time with proper attire.

ATTIRE: It is expected that all clients purchase at least (1) pair of flat/lifting shoes and (1) pair of mixed cardio/ or running shoes. Attire should not prohibit your ability to perform any of the physical requirements of each session.

If you need assistance with purchasing fitness shoes or attire please send a note.

FEE. The Client agrees to pay the Personal Trainer a fee of $_____________ for the Services on a per-session basis or monthly basis. he Personal Trainer requires the following payment instructions:

Payments must be made via member site invoicing and are due on either the 1st or 10th of the month.

PAYMENT TERMS & CONDITIONS

AUTO-RENEWAL: Following the initial 90-day membership term, this membership agreement will renew and automatically continue on a month-to-month basis at the program installment amount listed above. This auto-renewal can be canceled with a 30-day notice, submitted to Reggis Ball.

Any outstanding balances plus any monthly program installments scheduled during the 30-day notice period will be due in full at the time of cancellation to satisfy the cancellation policy.

SESSIONS: Sessions can be scheduled per week per the times below:

Monday: 4:00AM 6:00AM 8:00AM 10:00AM

Tuesday: 4:00AM 6:00AM 8:00AM 10:00AM

Wednesday: 4:00AM 6:00AM 8:00AM 10:00AM

Thursday: 4:00AM 6:00AM 8:00AM 10:00AM

TERM. This Agreement will start with the Session 2024 and continue for an ongoing period. The minimum term of the contract is 90 days( 3 Months) and you agree to with the option to continue until canceled by either Party with 30 day(s) notice ("Term").
