If vet care is needed, we will transport your pet immedicately to receive care. Vet fees are the responsibility of the pet owner.

Vaccinations up to date?*

A copy or picture of updated vaccinations will need to be provided. Required vaccines: Rabies, Bordetella (Kennel Cough), and Distemper/Parvo. We do accept Titer Tests showing proper immunity against the diseases mentioned above.

If your dog is an escape artist, it is highly recommended to have a GPS tracker attached to their collar. We make every effort to provide a safe environment for your dog without any methods of escape present.

Does your dog have anxiety? If yes, please be descriptive on what we need to look for and the best method we can help in any way.

If anxiety is from being around other dogs, our drop-in visits in your home will be a better option for care. Our environment is a very social one. Your dog must enjoy being around other dogs to thrive in our environment.

For your security, we will never post them while you are away. They will be posted at a later date after your return.

Are they on any medications, vitamins, ointments, or supplements? If yes, please explain quantity needed, when they need to be taken or applied, any special methods used to get your dog to take them (if necessary), or any other information that will make this process go as smooth as possible so we can make sure your pet gets what they need each day. Be very specific with dosage and how often.*

Feeding instructions. Provide how much you feed them (using cup measurements, ex. 1/2 cup, 1 cup, etc.) and how often.*

I hereby authorize the attending veterinarian to treat any of my pets and I accept full responsibility for all fees and charges incurred in the treatment of any of my pets.

The Pet Sitter is authorized to transport my pet(s) to and from the veterinary clinic for treatment or to request "onsite" treatment if deemed necessary. If I cannot be reached in case of an emergency, the Pet Sitter shall act on my behalf to authorize any treatment excluding euthanasia.
